Scheduling an Appointment
Patients
can call CRT Surgical Associates at 718-445-0220 to speak with one of
our clinical coordinators and schedule an appointment. When
calling the office, an automated phone menu helps direct callers to the
appropriate person. Patients will be asked about type of insurance, the
reason for referral, and the referring doctor. Patients who do not speak
English should have a family member attend the appointment whenever
possible.
What to bring to your office visit:

Insurance card(s), the office will make a copy to keep in your
chart.

Referral if needed. Please check with your insurance carrier to
see if you require a referral for a specialist visit.

Contact information from the physician who referred you;
including their name, address, and phone number. It may help to
have a referral slip or business card with this information.

Either all of the medication bottles that you are currently
prescribed, or a list of the medications and dosages.

Any films and reports.
-
Co-payment which is due at the time of your office visit.
What to expect when arriving at the CRT Surgical Associates:

First time patients:
Patients must check in at the Information Desk in the hospital
lobby. Our office suite is located in the West Wing, 300 Suite,
on the Basement Level. Upon arrival, the Receptionist asks
patients for their name, appointment time, and the name of the
doctor they are scheduled to see. Patients are also asked to
fill out a new Patient Information Form and are given a copy of
our Privacy Policy. The Receptionist will then copy the
insurance card and take any films and reports to prepare your
chart for the doctor. A Medical Assistant will then escort the
patient to the exam room. After the exam has been completed, a
follow-up plan (e.g., a follow-up appointment, additional tests,
etc...) will be discussed.

Returning patients:
Patients must still check in at the hospital's Information Desk.
Upon arrival to the CRT, patients should notify the Receptionist
of any changes in insurance, address, or phone number. Any films
or reports should be given to the Receptionist.
Parking
Parking is available on the street near the office or in the hospital
parking lot for a fee. If you are having a radiology procedure on that
day you can park in the hospital lot for a reduced rate
