New York Hospital Queens Diagnostic Laboratories is an outreach program of New York Hospital Queens, providing quality lab services for physicians, and clinics and in the New York area. New York Hospital Queens Diagnostic Laboratories maintains patient service centers in our geographic area to provide comfortable surroundings for routine adult and pediatric specimen collections. Individuals may present themselves to these facilities with either a New York Hospital Queens Diagnostic Laboratories test request form or a health care provider's prescription. Minors who require laboratory testing must be accompanied by a parent or legal guardian at the time of collection.

This peripheral service center is located in just off Main Street two block south of Northern Blvd.
There are three requisition forms from which tests are ordered. Most clinical assays are denoted on our clinical laboratory form. Tests for HIV require completion of the HIV requisition. To complete a requisition form, enter your name, the patient’s name, sex, date of birth and billing data in the appropriate boxes. Indicate tests requested by checking the box to the left of the test. If a test you desire is not on the form, please enter it clearly under the heading Miscellaneous Requests. Please be sure to fill out all information to insure smooth processing of the specimen, especially applicable diagnosis (ICD-9) codes.
Label each tube and container with the patient’s full name and medical record number or preprinted requisition number that is found on upper right-hand corner of the requisition form. Please make certain that all containers and or specimen tubes are labeled accurately.
Specimen pickup can be arranged without charge Monday through Friday between the hours of 8:00 a.m. and 10:30 p.m.. Call (718) 670-2574 to arrange for transport. Saturday and Sunday pickups available upon request and arrangement.
Most diagnostic tests can be completed and reported within a 48 hour period. All test evaluations are hand delivered or mailed to the client as soon as possible. Test results can also be obtained via phone by calling (718) 670-2574.
All clinical laboratory reports show the results with an accompanying reference range of normal values if sex and date of birth are provided. Critically abnormal values are reported immediately by telephone and tests are repeated at no extra charge. If a test result appears questionable, the laboratory will automatically repeat the procedure to validate reported values.
Patients are billed on a monthly basis. Please make sure that the requisition form is accurately filled out with the patient’s name, date of birth and billing address. Medicare patients who provide their numbers on the requisition form will have their bills sent to the Medicare agency. If your patient has insurance coverage, other than Medicare, New York Hospital Queens Diagnostic Laboratories will submit the claim directly to their insurance carrier if the following is clearly noted on the requisition:

Supplies needed for obtaining and transporting specimens are available at no extra charge. To order supplies, forward a supply order form or call the laboratory at (718) 670-2574. Available supplies are noted on pages 8 through 10. Supply orders are monitored so as to ensure that amounts requested are proportional to testing activity.
