During and after your stay at NewYork-Presbyterian/Queens, caregivers
and hospital personnel including physicians, nurses, technicians, case
managers, patient advocates, analysts, etc. may review your medical
records. Only personnel directly involved in patient care, payment
and/or hospital operations are authorized to access patient information,
pursuant to Health Insurance Portability and Accountability Act (HIPAA)
guidelines. NYP/Queens has taken precautions to ensure patient
confidentiality at all times.
Should you wish to have your information released to another physician, facility, etc. you may sign an Authorization for Release of Health Information Form (click to open). This form is also available through our Health Information Management Department.
You may also obtain a release form from your nurse, or by visiting the department, and someone will assist you with the completion of this form. The HIM Department is open from 9:00 a.m. - 6:00 p.m. M-F 9:00am - 5:00pm S-S and is located on the first floor room A198.
The NYP/Queens Patient Portal is now available should you wish to access your patient care information directly. If you have already provided us with your valid e-mail address, you may sign onto the Patient Portal
Click on Need an account? And enter your valid demographic information and you will be prompted to enter a username/password and set up an account.Once the account set-up is complete, you will receive an email that contains a link. YOU MUST CLICK ON THE LINK IN ORDER TO ACTIVATE YOUR ACCOUNT. You will have access to your problem list, medications, recent lab results, allergies and immunization information, via a secure website. If you need any IT support please call us at 877-448-1769. Should you need assistance with accessing the portal, or to have your e-mail information added to your record, you can also contact the Health Information Management Department directly at 718-670-1090. (The process of updating your e-mail information may take up to 24 hours before you can create and access your portal account).
