Practice administrative assistant to schedule an
appointment. When making the appointment, our staff will
inquire about your type of insurance, the reason for referral and the
referring doctor.

What to bring to your office visit

Insurance card(s)

Please remember to bring your insurance
card to your appointment. Also,
alert the office if your insurance
information has changed and needs to be
updated. Insurances

Workers' Compensation or No-Fault injury: please bring the name
of the insurance carrier, date of accident, adjustor name and
contact information as well as the case number.

Photo identification

Referral, if needed. Please check with your insurance carrier to
see if you require a referral for a specialist visit.

It is your responsibility to know if
your insurance has specific rules or
regulations, such as the need for referrals from
primary care physicians. If
your HMO/Managed Care Insurance Plan
requires you to obtain a referral from your
Primary Care Physician before you
can see our specialists, you MUST bring in
with you at the time of your
appointment. Please coordinate this
with your Primary Care Physician. Appointments
will be rescheduled if required referrals are
not brought in the office by the time of your
visit.

Co-payment, which is due at the time of your office visit

Your health benefit plan is an
arrangement between you, the enrollee, and the
insurance company, HMO or your employer.
Please remember that your co-payment is a
specific dollar amount you must pay to the
provider at the time of service. As a
contracted in-network provider in your insurance
plan, we are legally required to collect
this co-payment, as it represents your shared
cost of the treatment with your insurance plan.
What to expect upon arrival
First time patients
In order to expedite your check-in and minimize waiting times, we ask
that new patients arrive at the office 15 minutes prior to your
appointment. You may request that the paperwork be sent to you in
advance to bring with you to your appointment.
Upon arrival at 163-03 Horace Harding Expressway, patients may take
either elevator from the lobby to the 2nd floor and check in at the
registration/reception desk. The receptionists/registrars will
ask for your name, appointment time and the name of the doctor you
are scheduled to see. Patients will be asked to fill out our "new
patient demographics packet" while the receptionists/registrars copy the
insurance card as well as your photo identification. If an
x-ray has been scheduled for you, this will be done prior to seeing your
doctor. Following this, a Medical Assistant will then
escort a patient to the exam room. After the exam has been
completed, a follow-up plan (e.g., a follow-up appointment, additional
tests, etc.) will be discussed.

Patients please check in at the registration/reception desk.

Street parking is available near the office.
