ability to regain functional independence in activities of daily living
and functional mobility in the home, work and community while
maintaining physical, physiological and emotional health.

The staff will provide each patient with a comprehensive assessment and
examination to develop a specialized rehabilitative program to maximize
function. The clinician works closely with the patient to develop a
specific plan of care. Each patient's plan is closely supervised to
ensure maximum benefit and reach high therapeutic outcomes.
Appointment Process
A prescription for outpatient occupational therapy needs to be obtained
from a medical doctor to ensure medical necessity for the services. An
assessment by a skilled therapist will be performed to develop a
specific treatment plan. Please be aware that if your insurance company
requires a referral from your primary care physician, you must bring it
with you.

Appointment Hours
Effective Monday, January 1, 2012 Occupational Therapy hours for
patient care at our offsite location are as follows:
Monday to Thursday from 8am to 6pm
Friday 8am to 4pm
Appointments are made with the assistance of our administrative support
staff at 855-37REHAB(73422).
If you require language assistance, our department has various staff
members who are available for language interpretation. In addition, we
have a phone-based language interpretation service available.
