During and after your stay at NewYork-Presbyterian/Queens, caregivers
and hospital personnel including physicians, nurses, technicians, case
managers, patient advocates, analysts, etc. may review your medical
records. Only personnel directly involved in patient care, payment
and/or hospital operations are authorized to access patient information,
pursuant to Health Insurance Portability and Accountability Act (HIPAA)
guidelines. NewYork-Presbyterian/Queens has taken precautions to ensure
patient confidentiality at all times.
Should you wish to have your information released to another physician, facility, etc. you may sign an Authorization for Release of Health Information Form (click to open). This form is also available through our Health Information Management Department.
You may also obtain a release form from your nurse, or by visiting the department, and someone will assist you with the completion of this form. The HIM Department is open from 8:00 a.m. - 5:00 p.m. Monday to Friday and 8:00am - 4:00 p.m. Saturday and Sunday, and is located on the first floor.
