NewYork-Presbyterian/Queens has patient portals to make it easy for you to access your personal medical records at your convenience, right here on online.
If you were in the Emergency Room or admitted to the hospital and were given specific instructions on accessing the Inpatient and Emergency Room Portal upon discharge: click here.
If you already signed up for a patient portal at your doctor's office and were given a user name and password: click here.
Note: You may have, or need, access to both portals depending on your medical history.
If you were in the Emergency Room or admitted to the hospital:
Click on "First time here? Click to REGISTER." You will be asked
to enter valid demographic information and then prompted to enter a
username/password to set-up an account. Once the account set-up is
complete you will be asked to match your Hospital records to your
account. When prompted for additional information, include the
10-digit verification code located on your discharge paperwork.
Once complete, you will have access to your problem list, medications,
recent lab results, allergies and immunization information, via a secure
website. If you need any IT support or need assistance matching your

During and after your stay at NewYork-Presbyterian/Queens, caregivers
and hospital personnel including physicians, nurses, technicians, case
managers, patient advocates, analysts, etc. may review your medical
records. Only personnel directly involved in patient care, payment
and/or hospital operations are authorized to access patient information,
pursuant to Health Insurance Portability and Accountability Act (HIPAA)
guidelines. NewYork-Presbyterian/Queens has taken precautions to ensure
patient confidentiality at all times.
Should you wish to have your information released to another physician, facility, etc. you may sign an Authorization for Release of Health Information Form (click to open). This form is also available through our Health Information Management Department.
You may also obtain a release form from your nurse, or by visiting the department, and someone will assist you with the completion of this form. The HIM Department is open from 9:00 a.m. - 6:00 p.m. M-F 9:00am - 5:00pm S-S and is located on the first floor room A198.
