What to expect when arriving at the CRT Surgical Associates:
First time patients:
Patients must check in at the Information Desk in the hospital
lobby. Our office suite is located in the West Wing, Lower
Level, Suite 300 and can be reached by taking the Main
Lobby elevators to the Basement level. Upon arrival, the
Receptionist asks patients for their name, appointment time, and
the name of the doctor they are scheduled to see. Patients are
also asked to fill out a new Patient Information Form and are
given a copy of our Privacy Policy. The Receptionist will then
copy the insurance card and take any films and reports to
prepare your chart for the doctor. A Medical Assistant will then
escort the patient to the exam room. After the exam has been
completed, a follow-up plan (e.g., a follow-up appointment,
additional tests, etc...) will be discussed.
Returning patients:
Patients must still check in at the hospital's Information Desk.
Upon arrival to the CRT, patients should notify the Receptionist
of any changes in insurance, address, or phone number. Any films
or reports should be given to the Receptionist.
