Medical clearance is also known as medical evaluation.
Depending on your age and medical history, medical clearance to ensure you are in good health may be required for outpatient orthopedic procedures.
You must call your medical doctor and make an appointment with him/her
to review your pre-admission testing results and provide your clearance.
We suggest you make an appointment to see your specialist/physician
within one or two days after your visit to the PAC. This allows the PAC
time to fax them the results, and it gives your physician time to send
your clearance to the PAC. This medical clearance should be faxed to
both the PAC at NewYork-Presbyterian/Queens (fax number 718-661-7783)
and to your surgeon’s office. It is extremely important that your
clearance be faxed to the PAC so that it can be reviewed and placed into
your medical chart.
Most of the time, if additional clearances are necessary, you will be notified at the time you schedule the surgery. However, additional clearances are sometimes needed (such as are required by your primary physician in order for him/her to give the final medical clearance or as required during your evaluation during your pre-admission testing). According to your history or current condition, you may be required to be evaluated by a specialist such as a cardiologist, pulmonologist, or a neurologist. These additional clearances should also be faxed to the PAC.
