Update on NewYork-Presbyterian Queens DSRIP PPS Program

WHAT and WHY: This forum is an opportunity for journalists to learn more about health services provided by this Performing Provider System (PPS) program which is part of the New York State-funded healthcare transformation program known as DSRIP (Delivery System Reform Incentive Program).
Guest speakers include Valentine Hernandez, Executive Director,
Queens Coordinated Care Partners (QCCP), a health home, and Lisa Cole,
NAI Coordinator and Wai Ping Kan, outreach specialist from New Americans
Welcome Center, an agency developed by YMCA to help new immigrants
achieve health literacy, cultural competence, and self-sufficiency.
The NYP Queens PPS application was officially approved in March 2015. This is one of 25 plans accepted by the Department of Health for implementation in New York State. The NYP Queens plan addresses some of the most urgent health care needs of our community through integrated, high-performing health care delivery projects. The projects in progress involve primary care providers, specialty providers, hospitals, community-based organizations, and behavioral health providers.
Projects were selected for implementation based on findings from a comprehensive community needs assessment done in 2014. The nine new programs address mental health, asthma, cardiovascular and HIV treatment needs, as well as integration of palliative care, home care and other areas.
Plan development was collaborative, and input from the community was central to its creation. Many components were included to ensure that new programs would address community needs and that their outcomes would be appropriately reported.
NewYork-Presbyterian Queens, located in Flushing, New York, is a community teaching hospital affiliated with Weill Cornell Medicine, serving Queens and metro New York residents. The 535-bed tertiary care facility provides services in 14 clinical departments and numerous subspecialties. Annually, 15,000 surgeries and 4,000 infant deliveries are performed at NewYork-Presbyterian Queens. With its network of affiliated primary and multispecialty care physician practices and community-based health centers, the hospital provides approximately 162,000 ambulatory care visits and 124,000 emergency service visits annually.
